Creon should be taken with food according to my bottle's directions, and not before. I have been taking it for over three years. I take 1-2 with meals depending on how much fat is in the meal. The main purpose of the pancreatic enzymes is to digest fat. If I have a snack with a fair amount of fat I would also take one, but if it is just a piece of fruit I would skip it. Since I now have no pancreas, if I forget my pills I get reminded quite well the next day when that meal passes through (significant gas and diahhrea (sp?). Hope this helps - Robin F
