I've had a lot of success with my HMO, but it's required a lot of work. The first thing you need to do is find out if there is a facility in your tricounty area where they have experience treating pancreatic cancer. If you find one, request a consult there. If you don't find one, document your search (in writing), explain why you believe he needs to be seen by an expert, and handcarry the letter to the Plan Administrator. If you can get his oncologist or PCP to certify that he needs a consult with an expert, that will help a lot. In the 3 years since my dx with pancreatic cancer, I've had the best treatment available, but I had to write a lot of letters & restrain myself from yelling at people. Good luck
