re: medical release forms....I just typed one up on my computer stating ' 'mom's name' (DOB xx/xx/xx)gives permission for all medical forms, tests, results to be released to 'my name''

I don't remember how I worded it exactly - but it was something like that.

and mom dated and signed it. Her medical facility accepted it with no problem. I've made several copies w/ original signatures so if I need to get copies from anywhere else I would be able to. If need be you can have it notarized, but we didn't need to. Just ask the doc's office if they have a release there your dad can sign, or what they require. My mom's oncologist already knows I want copies of everything and usually has them ready for me,
