You sound so loving and compassionate. I am sure you will be able to tell her the news. But consider getting help -- either from her doctor (if he/she is someone that you feel can deliver the news in a gentle manner) or someone on his/her service. You might also want to contact Hospice -- just to get information. They provide all sorts of ancillary services and will most likely be able to steer you through this delicate subject. Maybe you need to ask your brother to come home when this happens, but I also understand how he may hinder (and not help) the communication process.
