Hello. You have to have the large surgery. Mine started as a sb-ipmn but eventually grew and moved into the main duct. If the cytology is good and it's not growing you may want to consider holding off on the surgery. I know it's worrisome to have it as I waited 4 years before having it removed but the surgery is pretty intense as well.
