John- You raise good questions. The sensitivity is low, but, I think adequate. The specificity is the real worry for any screening test (too many false positives causes harm), and the specificity, so far, looks great.
You are also right about the cost- the benefit of any test has to be balanced with the costs (which include not only the test itself, but also the costs of follow-up tests and patient worry).
