Does he have a Pain specialist or Palliative care doctor helping now?

Have they considered adding a Neuropathic pain medicine, combined with the fentanyl? This can be helpful, and can be increased over time. Examples include Neurontin, Effexor, Cymbalta - all of these have cheap generic forms.
