I'd try to hang tight at home if you can. Have the nurse come see her as frequently as possible and Monday call first thing in the AM and insist on speaking to the team at Columbia that manged her care last week. Be ready to bring her in if they can see her ASAP. Call every day until you get in.

If she gets a fever, looks suddenly worse, I would try to drive her to Columbia if you can. The home nurses should be a little helpful.

Good luck. Hope you can hang tight.

And make sure they start treating her depression. It is a symptom just like ascities, and few symptoms are so important.
