It might be possible but most PC is not found by feeling a lymph node. Lymph nodes can be swollen for many reasons not related to cancer.

If PC has spread beyond the organ to the lymph system, then yes, that is classified as stage 4. Generally surgery is not done at stage 4 though some patients after chemo have gone on to have surgery (but this is very case specific).

Diagnosis is best done with a ct-scan, ercp and blood tests in combination with each other.
