This is a very tough decision. My first impulse is similar to the other poster and to avoid surgery if at all possible. However, I would probably get multiple opinions from the best surgeons you could fine since it sounds like your poor husband is really suffering from all these obstructions. Sometimes there isn't an easy answer...

I would ask if it is possible to compromise and do a minimally invasive surgery (laproscopic) to minimize the chances of more adhesions forming. I would find the best minimally invasive general surgeon in your area... someone young-ish, who does this type of surgery all the time.
