Is the hot spot that lit up in the same place where your cancer was before? If so, then it is most likely cancer.

What is hard is sometimes hot spots show up in the intestines/stomach that can be inflammation (colitis from chemo, gastritis from gastroparesis with reflux) and hot spots in the bones can be arthritis.

But you are right that you can't know for sure without a biopsy. But if your recurrence was that large, they must have been able to see it on the CT + contrast as well, and it probably looked like cancer.
