Without a biopsy, you are never 100% sure and you have to be careful as some biopsy are not 100%. So, it boils down to their best educated guess and what they have seen/dealt with before.

Not to be too much of a downer but medicine is more art than science still. Go with the doctor's gut - if he thinks its cancer, then he's probably more right than wrong.
