It is a little odd to be speaking to the secretary. Are you calling his academic office instead of the clinic? Most of us call the clinic and ask to speak to his nurse and leave a message with the nurse, or with the answering service at the clinic. You can even ask them to page the attending doctor, if you want urgent reply.

I would try talking to the nurse, and ask them how/when can you speak with the doctor. Make an appointment for this/next week if you need to. Ask them what the system is to be able to reach a provider when you have a question.
