Thank you all for your replies. I learned a lot and gained a sense of hope. My husband has gained back three pounds and some muscle from working out at the gym. He feels stronger and almost ready for chemo. It turns out that my husband's bilirubin levels are not yet down to the level where he can start chemo anyways, so at most we will lose a week of chemo by going away. We leave tomorrow and are hoping for good weather in South Beach (Miami). On balance, I think we made the right decision going away before chemo for our specific situation. It's so great to be able to get input on these tricky quality of life decisions here. Doctors just can't or won't help you with these types of decisions.
