There are two surgeons I know who are aggressive and have experience with surgery in this area. At USC in Los Angeles is Dr. Rick Selby. The other is at St. John's Medical Center in Santa Monica. Dr. Gagandeep Singh is with the John Wayne Cancer Institute there.

Both are extremely experienced and skilled in the GI area as both have done liver, kidney, pancreas transplants besides removing cancer. Transplants involves cutting veins and arteries and reattaching them. May be worth a try. If you need contact info, please let me know. Also Google their names and you will find numbers for both. Both of these doctors/surgeons have a good bedside manner and can be very aggressive.
